Designate a Personal Representative
Authorize another individual to act on your behalf where permitted by law.

File a Complaint
File a complaint without fear of retaliation if you believe your privacy rights have been violated.

BenefitsAssist reserves the right to revise this Notice and make the revised Notice effective for information already maintained as well as information received in the future.
